At Town Farm, we recognise and value the importance of strong partnerships between home and school to support pupil well-being and academic success. Our Home-School Link Worker plays a vital role in building these connections, providing guidance, and offering support to families.
Our Home-School Link Worker is here to assist parents and caregivers in navigating any challenges that may affect their child’s learning, behaviour, or emotional development. Whether it's offering advice on attendance, addressing concerns about well-being, or providing links to additional resources, our dedicated Home-School Link Worker is a key point of contact for families.
We encourage all parents to reach out with any questions or concerns. Together, we can create a nurturing and supportive environment for our children to flourish both in school and at home.

My name is Amy Chilton and I am the Home School Link Worker for Town Farm Primary and Nursery. I am also responsible for the Attendance & Welfare of children who attend the school.
My role as Home School Link Worker is to support parents with any worries or concerns they may have about their children or home circumstances while they are on roll at this school.
I have contacts with a number of outside agencies which are available to support families.

Monday & Tuesday 8am-4pm & Wednesday, Thursday & Friday 8am-3.30pm.
Who can I help?
Families who are experiencing difficulties in managing their child’s behaviour.
Families whose children are not attending school or who have been excluded.
Families of children who are reacting to change, such as parental separation or bereavement.
Families who may need support on a wide range of issues impacting on their family life, e.g. illness, disability, separation, domestic violence, bereavement, financial issues, housing.
Families who need support in dealing with other agencies or help and advice to complete forms such as school admissions, free school meals, special education needs, housing, benefits etc.
Staff at school can ask for my involvement. With your help I will explore the difficulties your child or family is experiencing and together try to find the best way forward. I may therefore contact you if school staff have asked me to.
